Important note: Your claim form CANNOT be filed electronically. You can use the SIPC Claim Form Online Center to fill out your form and print it out. However, you must still mail the completed form and all required attachments to the designated address. Your claim form is considered filed only when it is RECEIVED in total by the designated party.

Please note that you can fill out a claim form only if your broker-dealer is in liquidation under the Securities Investor Protection Act. If you are unsure whether your broker-dealer is in liquidation, click on the arrow under "Identify Your Brokerage Firm" below or contact SIPC.
