Assessment Forms SIPC-6 and SIPC-7, to be used for the net operating revenue assessment which began April 1, 2009.
SIPC-6 General Assessment Payment Form: To be filed for the first half of the fiscal year.
SIPC-7 General Assessment Reconciliation: To be filed at the end of the fiscal year with a place to deduct assessment due and paid as reflected on the SIPC-6 filed.
